Body Doubling: Why It Works for ADHD and How to Use It

What body doubling is, why it works for ADHD task initiation, what the evidence does and does not show, and how to use it without overcomplicating it.

Body doubling is the practice of working alongside another person, in person or virtually, while each of you does your own thing. The other person is not helping you with the task. They are simply there. For many adults with ADHD this is one of the single most effective task-initiation tools available, and it is largely free. This article covers why it works, what the evidence does and does not show, and how to use it without overcomplicating it.

What body doubling actually is

The basic shape is simple. Two (or more) people set up to work at the same time, in the same physical space or on a shared video call, on their own separate tasks. There is usually a short opening (what each of you intends to do), then a defined block of work (often 25 to 90 minutes), then a short closing (what got done). The work itself is silent or near-silent.

The other person is not a coach, a teacher, an editor or a manager. They are a body in the room, ideally focused on their own thing. The structure does the work.

Variations are everywhere:

  • A friend or family member working at the same table
  • A scheduled video call with a colleague or a friend
  • One of the paid coworking platforms (Focusmate, Caveday, ADHD coworking apps)
  • An online study or coworking stream
  • A coffee shop, library or public workspace

The format is more flexible than its many proprietary versions suggest. You do not need a paid app.

Why it works

The mechanism is consistent with what is known about ADHD and arousal. Adult ADHD involves chronic under-stimulation of the regulatory systems that switch attention on for low-stimulation tasks [1, 2]. Boring tasks fail to register; the brain looks for something more interesting; the task remains unstarted. The well-described "optimal stimulation" model in the wider attention literature, drawing on work in the 1980s by Zentall and others [3], captures this picture: arousal that is too low for the task at hand is the problem to solve.

Body doubling raises arousal through several routes simultaneously:

  • Mild social presence activates the same systems that meetings activate, without the cost of having to perform in one
  • Time structure (the block has a start, a length and an end) substitutes for internal time sense
  • Public commitment at the opening makes the task externally visible
  • Reduced switching cost because the other person also expects you to stay on this task
  • Reduced isolation for the lonely flavours of executive function (admin, household tasks, writing)
  • Background variation in the room that is enough to keep the brain interested without being distracting

The combination is more useful than any single component. Each on its own (a deadline, a video call, public commitment) helps; together they regularly produce work that would not otherwise happen.

What the evidence does and does not show

Body doubling has substantial anecdotal support from adult ADHD communities and is widely recommended by coaches and ADHD organisations [4]. There is currently limited formal randomised-trial evidence specifically on body doubling as a named intervention; what exists sits within the broader literature on contextual support, social facilitation and scaffolded executive function. The Faraone consensus statement and Barkley's executive function model both support the underlying mechanism even where the specific named practice has not been the subject of large trials [1, 2].

This is worth being honest about. Body doubling is supported by mechanism, by widespread adult-ADHD-community practice and by clinical experience. It is not yet supported by a deep stack of randomised controlled trials. The case for it does not depend on RCTs the way the case for medication does; the cost is essentially zero, the practice is harmless, and the lived-experience signal is strong.

How to use it

Several practical points consistently emerge:

Match the task to the format

In-person body doubling is best for physical tasks (admin, paperwork, household chores, packing). Virtual body doubling works well for screen-based tasks (writing, emails, design, study). Match the format to what you are doing.

Keep the opening short

The opening should be 30 seconds to two minutes. What you are doing, how long for. Long openings turn into avoidance.

Use a fixed block length

25 minutes (one Pomodoro) is the minimum useful block. 50 to 90 minutes is the sweet spot for most adults with ADHD. Open-ended blocks tend to dissolve.

Set up reliably, not optimally

A regular weekly slot with one specific person is worth far more than a perfectly designed system that you never actually start. Reliability is the variable. Pick the easiest combination you will keep doing.

Be honest about who works as a body double

Some friends are good body doubles; others are not. The good ones are people who can actually focus on their own thing in your presence. People who want to chat, who keep checking in, or who finish their task in five minutes and then watch you, are not body doubles; they are interruptions.

Use the closing

A short closing (what got done, what is next, when you will body double again) cements the session. Most useful is the simple fact of saying it aloud.

Combine with medication and structure

Body doubling is one tool among several. It works best where medication (where indicated) and a basic working system are also in place. It does not replace either; it sits on top of both.

When body doubling does not help

A few situations where it is not the right tool:

  • The task requires deep, silent concentration with no human presence at all (some types of writing, some types of coding)
  • The other person is socially demanding and you are spending energy managing them
  • The activity is one where you genuinely should be giving your full attention to the other person (parenting, caring, relationship time)
  • The setting is so distracting (loud cafe, busy household) that it costs more than it gives

In each case the problem is not body doubling; it is a mismatch between body doubling and the situation.

What this means in practice

  • Body doubling means working alongside another person, in person or virtually, while each of you does your own thing. The other person is not helping with the task.
  • It works because it raises arousal for low-stimulation tasks through social presence, time structure, public commitment and reduced switching cost.
  • The evidence base is grounded in mechanism and lived experience more than in randomised trials; the cost is essentially zero and the practice is harmless.
  • Match the format to the task (in-person for physical, virtual for screen), keep openings short, use a fixed block, and prioritise reliability over optimal design.
  • It is one tool, not the tool. It works best alongside medication where indicated, basic structure, and friction-reducing design at the start of the task.

When to speak to a professional

Speak to your GP if ADHD has not been formally assessed and task initiation is being significantly affected. Where the NHS adult ADHD wait is not workable, private adult ADHD assessment is a legitimate parallel route. For structured ADHD coaching that often includes body doubling as part of the work, Access to Work is the usual funding route. Seek urgent help via 111, 999 or A&E for any acute mental health crisis or significant safety concern.
