What the ADHD Passport is
ADHDassess comes from the family behind NeuroFX: Neuro FX Ltd runs both. The Passport exists for the stage before an assessment: when you, or your child, may have ADHD and you want to put the picture together properly before deciding what to do next.
You build it yourself, in your own words and at your own pace. You type or speak your answers, save and come back, and invite someone who knows you well, or a teacher, to add their view. For adults, the questionnaires are the ASRS-v1.1 and BAARS-IV, with mood, anxiety and sleep measures (PHQ-9, GAD-7 and ISI) where they apply. For children aged 6 to 17, a parent completes SNAP-IV ratings, with optional teacher ratings.
Building your Passport is free. No card, no subscription. You complete the free part in full, review it, and only then decide whether you want the clinician-reviewed screening report.
Who it is for
- Adults aged 18+ who are not sure an assessment is the right next step, or who have just done one of our free screeners and want something more careful. The adult Passport, explained.
- Parents of children aged 6 to 17 who want to bring their own observations and the school's together before a conversation with the SENCo or the GP. The child Passport, explained.

What is free, and what you pay for
Building the Passport costs nothing: your story, your history, the questionnaires and any informant contribution. The clinician-reviewed screening report, your finished Passport, is £99, paid once. A NeuroFX clinician reviews it before release. It is one digitally certified PDF: an introduction letter, a clinical summary, your screening findings and any informant evidence. It is normally ready within five working days of payment.
You can share it with your GP, school or SENCo, university, employer, therapist or assessment provider. Everything the finished Passport includes, with complete fictional examples you can read before deciding.
It is a screening and pre-assessment tool, not a diagnosis. It does not guarantee a referral, treatment, funding or adjustments. A diagnosis of ADHD can only come from a comprehensive clinical assessment with a qualified clinician.
The code towards a NeuroFX service
Every Passport includes a £50 code towards an eligible NeuroFX assessment or medication service. One code, one use, valid for 90 days from issue. It is issued after the Passport is bought, it is not cash, and it does not reduce the price of the Passport. If the Passport is refunded, the code is cancelled.
You do not need a Passport to book with NeuroFX, and the code is not a reason to buy one. £5 from every Passport goes to ADHD UK. Payment options, including instalments where offered, are set out on the ADHDassess pricing page.
How it relates to a NeuroFX assessment
A NeuroFX assessment is the route to a diagnosis: a full clinical interview with Tina (2 to 3 hours, with breaks), validated diagnostic tools, and a written report that your GP can act on. The fee is £675, fixed. Nothing about that changes.
The Passport sits before it. It is screening evidence, prepared by you and reviewed by a clinician, so that whoever you see next, including us, starts with a clear written account of your story, your screening findings and the everyday impact. It does not shorten the assessment, and you do not need one to book.

What is free, and what costs £99?
